Virginia law requires that all graduate and undergraduate students submit immunization documentation. A hold will be placed on your account and a late fee assessed if you do not submit your complete immunization records by the posted deadline.
As a student registered for classes at George Mason University, you must be fully compliant with Mason’s and the Commonwealth of Virginia's immunization requirements. If you are lacking requisite immunizations, you must either provide proof of receiving all necessary vaccines, or take these vaccinations from Mason’s Student Health Clinic, your health care provider, or the health department.
Students are strongly encouraged to meet these requirements at home and return the completed form as soon as possible. The forms can be faxed or mailed to the Immunization Office.
